This form may be used for copay reimbursement only if you are out a full day for personal treatment by a physician due to an accidental injury or illness and no other disability claim is filed.

Packets with vouchers are sent by mail to all those participating. If you did not receive your packet, please contact the Flex Department at 800-523-8422 to request another packet.
- 2009-2010 403(b) Employee Newsletter
- Employees may elect to open 403(b) annuities and may contribute a maximum of $15,500 per year. If interested, contact one of the approved annuity firms in the packet provided below. Once you have contacted an agent of your choice and your account has been set up, complete the deduction form in the packet and fax it to First Financial at 800-298-7785. The district will begin deducting funds from your paycheck once your application is processed. 403(b) Packet
- Please be aware that the IRS has changed the rules regarding Rev. Rul. 90-24 transfers from Section 403(b) annuity accounts administered through our benefits program. These rules affect account balance transfers made after September 24, 2007. We are currently developing a plan to address these new changes, which must be in place by January 1, 2009 as required by the IRS. However, until this issue can be addressed through our plan, we will not approve transfers or exchanges made on or after September 25, 2007 from these accounts.
PLEASE NOTE THAT IF YOU ATTEMPT TO TRANSFER FUNDS FROM ONE 403(b) ANNUITY OR INVESTMENT ACCOUNT TO ANOTHER AFTER SEPTEMBER 24, 2007, IN A MANNER THAT DOES NOT COMPLY WITH THE NEW REGULATIONS, YOUR ENTIRE 403(b) FUND BALANCE COULD BE CONSIDERED BY THE IRS TO BE TAXABLE INCOME TO YOU.
